Muscle atrophy and muscle sparing with bone resection of the femoral head (FSB) may be one of the acceptable methods of managing early-stage femoroacetabular impingement (FAI). Controversy exists regarding the surgical management of early-stage FAI with muscle atrophy. Controversy exists regarding the usefulness of FSB. We evaluated the efficacy of FSB. In this retrospective study, 105 patients (112 hips) with a mean age of 27.9 years (range, 12-63 years) with FAI and muscle atrophy were treated with FSB. All patients underwent surgery using the modified Watson Jones approach. The study population was divided into 2 groups: group A (n = 69, 76 hips) included the patients with muscular atrophy associated with intact labrum or unstable labrum, and group B (n = 43, 44 hips) included the patients with muscle atrophy and unstable labrum. The mean follow-up period was 45.1 months (range, 12-83 months). All patients with FSB had an anatomic reduction of the femoral head. With respect to the range of motion, hip flexion and external rotation increased in group A after surgery, compared with before surgery, and in group B after surgery, compared with before surgery. In group A, group B, and in the entire group, the hip center-edge angle decreased postoperatively, compared with before surgery; however, the angle at final follow-up showed no difference from the preoperative value.
